More than 1,000 people are dead. Over 2,400 are sick. And health officials have no idea where more than 80% of new Ebola cases are coming from. The ongoing outbreak in the Democratic Republic of Congo is not under control. Not even close.
The Number That Should Be Keeping You Awake
Here's the stat that infectious disease experts are losing sleep over: more than 80% of new Ebola cases in the DRC are appearing outside of any known contact chain. That means health officials aren't just failing to get ahead of this virus. They're not even seeing most of it.
As NPR reports, contact tracing is the central weapon in any outbreak response. You find a sick person, you identify everyone they touched, you monitor those people, and you cut the chain of transmission. It's labor-intensive, but it works. When it's working, most new cases should be coming from people already on your radar. The region at the center of this outbreak is dealing with a brutal combination of factors working against containment. The World Health Organization has announced it is training 21,000 community workers right now, which is encouraging in theory. People Are Hiding From the Clinics. Can You Blame Them?

As Boghuma Titanji, an infectious disease physician at Emory University, explained to NPR, people with symptoms are "going underground." They're seeking care from traditional healers. They're staying home with family. The stigma around Ebola is severe, and the mistrust toward outside medical investigators in this region runs deep. That is not irrational. That is a historically informed response from communities that have not always been treated well by the systems now asking for their cooperation.
Titanji described the situation to NPR as seeing only the tip of an iceberg, with no way to gauge the size of what's below the surface. That is a genuinely terrifying way for a physician working an Ebola outbreak to describe their visibility into the crisis.
The One Piece of Actual Good News
There is a thin silver lining, and it is worth stating clearly because this story does not have many bright spots. Clinical trials have begun for both treatments and a drug that could protect exposed people from developing the disease, according to NPR.
There is no approved vaccine for this strain of Ebola. That context matters. So the start of even experimental trials represents something real. Bhadelia told NPR that delivering better care through these trial therapies could be a tool for building trust with communities that are currently avoiding the health system. The logic is straightforward: if showing up to a clinic might save your life or protect you after an exposure, more people might show up.
Access is still extremely limited since the treatments are only available through clinical trials. This is not a solution. It is a potential wedge toward one, if the response moves fast enough to matter.

It means people are dying of a hemorrhagic fever in their homes, surrounded by family members who then become exposed, in communities that have decided, based on lived experience, that the official response is not for them. It is the downstream consequence of decades of exploitation, poorly managed previous outbreak responses, and active conflict making the region ungovernable for basic public health work.
The U.S. government, it should be said, is currently in the middle of gutting the exact international health infrastructure designed to respond to situations like this one. Cuts to USAID, hollowing out of CDC global programs, and the general posture of treating global health funding as foreign-aid waste product have consequences. Those consequences look a lot like a virus killing a thousand people with no clear ceiling in sight while health workers describe being unable to see the iceberg.
This outbreak is not a distant abstraction. Ebola with compromised contact tracing, in a region with transient workers and active conflict, in a world with international air travel, is a problem that does not stay neatly inside borders. The people dying in the DRC right now deserve better than a global health architecture that's being dismantled in real time for domestic political points. The math here is not complicated. It is just very, very ugly.
